As a Social Worker or Social Services Designee , you will provide emotional support and advocacy for residents and families, assist with care planning, and help address psychosocial needs throughout the resident experience. You’ll work closely with the interdisciplinary team to ensure smooth admissions, ongoing support, and effective discharge planning.
