Crisis Care Coordinator
Behavioral Health Services
Salary Pay Range: $52,769.60 - $79,102.40

Job Summary: Crisis Care Coordinators provide crisis stabilization services and linkage to community based services to clients in need of immediate support for a period of one to six months. Individuals are identified by Crisis Services as in need of further stabilization and/or an individual awaiting placement into a long term level of care. Services are provided in the community, at locations such as the person's resident, doctor's clinic, etc.

Essential Responsibilities:
  • Delivers Crisis Care Coordination to individuals until formal services can be obtained or the consumer has stabilized. This may include assistance with benefits, linkage to other services, discussion of stressors, and includes documentation as outlined in state guidelines.
  • Complete an assessment and crisis plan for the individual that includes collaborating with referral sources and developing individualized client care plans. Complete crisis progress notes that promote a person's recovery goals and identifies the needed response plan.
  • Attend any necessary staffings, community meetings, advocacy, etc. Complete warm handoff to a long-term agency that will be serving the individual when applicable.
  • Actively participate in weekly clinical supervision as per DHS 34 standards. Follow up as needed on recommendations made during supervision and document the supervision and follow up as per procedures.
  • Perform all other duties as assigned.
Minimum Requirements:
  • Current Resident of Wisconsin.
  • Possession of a valid Wisconsin Driver’s License, able to be maintained throughout employment.
  • Possession of a Bachelor’s Degree or higher in Human Services, Psychology, Social Work, or Occupational Therapy.
  • At least three (3) years of Human Services experience.
  • Intermediate knowledge of all related computer and software applications, such as word processing and spreadsheets.
  • Willing and able to travel up to 75% of the time in this role.
Preferred Qualifications:
  • Possession of LPC or LCSW licensure.
